All documents within the card signing packet must be completed within 24 hours. At the time of submitting, you must pay for your equipment package. (New or Returning player package available) You must submit payment using a credit card or banking information. You will not be able to complete the card signing process without paying for the equipment. The form of payment used (the account on file) will be the same account used to deduct the player fee installments. You may change or update anytime throughout the season.
YOU MUST COMPLETE THE PLAYER PACKET IN ITS ENTIRETY.

The payment structure is based on 20 players and is payable in 9 equal installments. The first player fee installment will be electronically deducted from the account on file on June 15th, 2026. The remaining payments will be deducted from July 2026 to February 2027.
Payment dates: 6/15/26, 7/15/26, 8/15/26, 9/15/26, 10/15/26, 11/15/26, 12/15/26, 01/15/27, 02/15/27.

HOME ICE ARENA:

LEAGUE:
Girls 12U - TBD
Girls 14U, 16U & 19U, No League Affiliation
Independent game schedule with core games against top ranked teams in North America. We will select a competition that will challenge our players.

GAMES:
Schedule TBD
Home games may occur during the evening throughout the week (when playing Illinois Teams) or any time on the weekends. Away Games could begin on a Thursday or Friday dependent on out-of-town Showcases/Tournament start day. Many out-of-town showcases / tournaments begin Friday mornings.

USA HOCKEY FEE:

OFF-ICE TRAINING:
Approximately twice per week, provided by High Performance Institute, a division of Illinois Bone and Joint, and their certified trainers. (Included in fees)

VIDEO REVIEW:
Coaches will provide video at their discretion/video room will be provided at Rosemont Ice Arena

GOALIE INSTRUCTION:
Led by Goalie Instructor Amanda Dinella

MENTAL SKILLS COACH:
Max Cook – Pre – Licensed Professional focusing on the athlete and improving their mental game by setting goals and the process of achieving said goals. Played hockey for Chicago Mission and Division 1 college hockey at Miami University.

COLLEGE COORDINATOR Applies to Girls 14U-19U
Tom Mandarino will oversee all communication with colleges, coordinating all aspects of player advancement from grades and transcripts to college visits. We take great pride in our in-house college placement advocacy and its success rate.

SLUSH FUND:
TBD Slush fund required to cover out of state Showcase / Tournament registration cost. Head coach and/or Manager oversee slush funds. Slush fund amounts vary per level and based on travel schedule. Slush fund payments are mandatory. In the event slush funds are not paid directly to Team Manager, Mission will add it to your player fee balance.

Acknowledgment

By signing the player card and fact sheet, you agree to pay all financial obligations. A player who may be injured is still obligated to continue payments. If the 2026-2027 Mission season is postponed with the ability to continue, payments towards fees will be processed as scheduled. If the season is canceled, a prorated refund or credit will be issued based on the length and services provided through cancellation

Agreement*

I have been fully informed of all the terms and conditions, including financial, required to play on the Chicago Mission team. If financial default occurs without notice or effort to pay, the player may be removed from all team activities. In the event the season ends and a balance remains, a financial hold will be submitted to our affiliates, blocking player release.
